Women's Health and Cancer Rights Act

These services include:

* Reconstruction of the breast upon which the mastectomy has been performed;
* Surgery/reconstruction of the other breast to produce a symmetrical appearance;
* Prosthesis;
* Physical complications during all stages of mastectomy, including lymph edemas.
 

The plan may not:

* Interfere with a woman’s right under the plan to avoid these requirements;
* Offer inducements to the health provider, or assess penalties against the health provider, in an attempt to interfere with the requirements of the law.
 

However, the plan may apply deductibles and co-insurance requirements consistent with other coverage provided under the plan.

Things to Remember

* Show your identification Card every time you go to the doctor.
* Be sure that all the providers (i.e., labs, x-rays, specialty physicians, etc.) participate with your plan.
* Emergency Rooms are meant for accidents or life threatening emergencies only!
* If you acquire or lose a dependent during the year, you must notify Human Resources within 30 days and complete the appropriate paperwork in order for the coverage change to take effect during the plan year.
